When contacting us to schedule your patients, please provide the following information:
- Reason for referral
- Type of specialist needed (Orthopedic Surgeon, Pain Management, etc.)
- Preferred provider or First Available
- Patient demographic and insurance information
- Corresponding records faxed to Physician or Clinic
We will work with your patient to complete the registration process and schedule an appointment at their convenience.
